If you're a new client, please download, read and sign the following forms and bring them to your first session.  If you have any questions or concerns, someone will be happy to go over these with you when you come in.  Please arrive approximately 30 minutes early if you are coming for your first session so there will be adaquate time to do this.

If you would like us to be able to speak to a family memeber, probation officer, attorney or to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), please complete this form to authorize release of information:
